No, Titanium Man isn't really that tall compared to Iron Man. Yes, they're really both standing there in one photo, without Photoshop. Tits is standing on something or other to make him bigger. We do the same thing with the regular Figuretoons, to make the OAFEs look similarly sized when they're all in a shot together - that's why you never see their feet. The scene is, of course, inspired by the meeting of Rocky Balboa and Ivan Drago. The background is even taken directly from the scene where they stand like this.
